- Please confirm all information PRIOR to us filling out any documentation so we can fill it out correctly the first time.
- YOUR INFORMATION: full legal name with correct spelling, address (in B.C.), address of destination, contact information (phone number[s] and email).
- PATIENT INFORMATION: full legal name with correct spelling, birth date (if birth date is unknown, please keep it consistent with all other documents you already have for your pet), microchip and/or tattoo (identification) number, vaccine records, etc.
- If any of this information provided is incorrect and the forms need to be filled out repeatedly, each repeat will incur an additional fee. To avoid any confusion, filling out the documents prior to arriving will make this process as quick and easily as possible. If any information on these documents is incorrect, or if any requirements are missing, due to your lack of research prior to the appointment, another appointment fee and document fee will be charged. Your forms will not be refunded if travel plans change or if they are denied by government official veterinarians.
